Hi, 26 year old
For 9 months now, crawling sensation/tingling on top of head and back of head and also on outside of right arm.
Sharp stinging pains on top of head left side
Have been on amitriptyline for 4 months but not helping.

Your symptoms need an MRI of the brain and neck spine and nerve conduction velocity study.

Your symptoms can be due to;

1) Pinched nerves in the neck

2) Occipital neuralgia.

The treatment steps are;

1) Conservative treatment;

a) posture correction

b) medications; tricyclic antidepressants, serotonin reuptake inhibitors, carbamazepine, oxycarbamazepine, gabapentin, pregabalin, and opioids.

c) Infliximab has shown some benefit,

d) Physiotherapy.

2) Local anesthetic agent injection with steroid

3) Botulinum toxin infiltration

4) Pulsed radiofrequency (PRF) treatment.

5) Occipital nerve stimulation

6) Neurolysis.

7) Other surgeries; C2 gangliotomy, C2 ganglionectomy, C2 to C3 rhizotomy, C2 to C3 root decompression, and distal neurectomy.

The crawling/tingling sensation is on top of my head and near the back of headOn the outside of my right arm
And sometimes on my right shoulder bladeI also have shooting/sharp pain in my head that comes and goes through the day.The tingling is constant daily

Following investigations are needed to exactly ascertain the cause;

1) Electromyography/nerve conduction studies (EMG/NCS)

2) MRI of the spine and brain

3) blood glucose and glycosylated hemoglobin

4) serum B12 level with methylmalonic acid (with or without homocysteine),

5) serum protein electrophoresis

6) Antinuclear antibody

7) Lyme testing

8) Rheumatoid factor

9) Sjogren's syndrome testing (Anti-Ro, Anti-La Antibodies)

Okay, thanks. I’m just really worried about a brain tumor as google says skin crawling /tingling can be caused by a tumor

It is quite unlikely.

A brain tumor causes headache, nausea, vomiting, vision issues, and limb weakness/paralysis.
