Which lymph nodes are enlarged?
You need the following investigations to ascertain the cause of your symptoms;
1) Cultures for bacteria, mycobacteria, and fungi (blood, urine, stool, sputum).2) Serologies for Brucellosis, CMV infection, amebiasis, toxoplasmosis, and chlamydial diseases.3) Ultrasound of lymph nodes4) Serum ferritin levels are useful in cases of SLE flare, and adult Still disease.5) Antinuclear antibody titers, rheumatologic factor, thyroxine level, and ESR,6) High-resolution CT scan of lung and CT scan of the chest.7) Ultrasound and CT scan of the abdomen.8) Technetium bone scan9) Peripheral blood smearThese will help in the diagnosis and ascertaining the cause of your symptoms.

Your symptoms are most likely due to a chronic infection.
The body fluid cultures can only tell the organ or system affected by the infection.
The other possibility is an autoimmune disorder which can cause fever, lymph node enlargement, and joints/back pain.
