Hello, my dog has not been himself for the last couple of days, he's off his food and is very lethargic, he's usually a very energetic dog so this is out of character

The changes that you have noticed with him are very concerning.

Inappetence and lethargy are fairly non-specific though.

Dr. Susan :

Can you lift his lip and look at his gums? What color are they?

OK. Is he having any increased effort to breathe?

Customer: No
Dr. Susan :

OK. Has he had any vomiting or diarrhea?

Customer: He vomited this morning

Excellent. Have you given any people food? Any diet changes recently?

Customer: We went away to the seaside last week & he hasn't been the same since we've been back, I gave him some liver yesterday as he was off his food

Since you were at the seaside, we can see unusual ingestions (such as sand) or water borne parasites like giardia.

Dr. Susan :

I recommend having him evaluated by your local vet. I would start with a physical exam, stool sample to look for parasites, and x-ray to look for foreign material or evidence of obstruction.
