I'm sorry to hear this. Unfortunately UK law prevents online prescribing of veterinary medicines. Your vet should be able to provide you with such a prescription if you ask them? They are not allowed to unreasonably deny a request for a written prescription.

Usually most vets will charge £10-£15 for a written prescription, unfortunately online pharmacies often can sell the drugs for less than what the vets can even buy them for, it is not so much your vet wanting to rip you off. Just economies of scale unfortunately.
A 3.4kg dose (the syringe should be calibrated in kg, not ml) would be the maximum dose she can have of metacam. Often cats with this condition suffer from concurrent secondary urinary tract infection, has she had a urine culture performed recently?
OK great. Sounds like you should be able to obtain a definitive diagnosis and also see if there is concurrent infection, in which case antibiotic will also be beneficial.
